This plugin allows you to insert a login form anywhere in your blog (header, sidebar, etc.) by adding to wherever you want the code to be generated. If the user is logged in, it will give them the option to logout. I've tested the plugin in WP 2.1.2, and you can see a working example of my styled version by clicking “login” just below the header of my site. The code is originally used in a hack offered by Teddy Hwang. I modified it a bit, and turned it into a plugin so it's easier to add to a WordPress blog.

I need a copy of “Login-Anywhere� Plugin and unfortunately the developer’s website has been down. Do you mind hooking me up with a copy of the plugin.
